Description
What It Is
Serelose contains lactulose (3.35 g per 5 ml), a non-absorbable osmotic laxative that helps soften stools by drawing water into the colon. It’s commonly used for:
-
Chronic constipation
-
Management of hepatic encephalopathy by reducing blood ammonia levels
How It Works
Lactulose undergoes fermentation in the colon, producing organic acids (like lactic acid) that:
-
Increase osmotic pressure, drawing water into the bowel (softens stool, stimulates peristalsis)
-
Acidify the colon contents, converting ammonia (NH₃) to ammonium (NH₄⁺)—reducing its absorption and alleviating hepatic encephalopathy symptoms
Indications & Uses
-
Constipation (Adults): Start with 15–30 ml once daily, increasing to up to 60 ml if needed.
-
Hepatic Encephalopathy (Prophylaxis): 30–45 ml every 6–8 hours, adjusted to achieve 2–3 soft stools per day.
-
Acute Cases: Up to 30–45 ml hourly until soft stool achieved, then maintenance dosing.
-
Children: 1–3 ml/kg/day (max 60 ml/day), adjusted for stool frequency.
Safety & Side Effects
-
Common: Bloating, flatulence, abdominal cramps (usually mild and temporary)
-
Overdose: Diarrhea, dehydration, and electrolyte disturbances (especially low potassium)
-
Contraindications: Galactosemia, intestinal obstruction
-
Precautions: Use cautiously in lactose intolerance or diabetic patients; dose may require adjustment
-
Generally safe during pregnancy (Category B) and breastfeeding, but follow medical advice
